Helpful Information for Individuals and Families

Navigating services for individuals with intellectual and developmental disabilities can often feel overwhelming. At Guiding Point Care, we are committed to helping individuals and families better understand the resources, programs, and services available to them.

This page provides helpful information and links to trusted organizations and programs that support individuals with developmental disabilities and their families.

Our goal is to make it easier for families to access the guidance, services, and information they need.

New Jersey Division of Developmental Disabilities (DDD)

The New Jersey Division of Developmental Disabilities provides services and support to eligible adults with intellectual and developmental disabilities.

DDD services help individuals live as independently as possible while receiving the supports they need to thrive.

Through DDD, eligible individuals may gain access to services such as:

• Support coordination
• Community-based services
• Employment supports
• Day programs
• Residential supports
• Behavioral and therapeutic services

Families can learn more about eligibility requirements and available services through the official DDD website.

Community Care Program (CCP)

The Community Care Program is one of the primary programs through which eligible individuals receive services funded by the state.

The program allows individuals to access a wide range of supports while remaining active members of their communities.

Services available through the program may include:

• Community-based support services
• Skill development programs
• Employment support
• Transportation services
• Residential services

Support coordinators help individuals and families navigate these services and ensure that appropriate supports are in place.

Finding the Right Services

Selecting the right services and providers can be challenging. Each individual has unique needs, goals, and preferences.

Guiding Point Care works closely with individuals and families to help them:

• Understand available services
• Explore provider options
• Develop a person-centered service plan
• Connect with qualified service providers
• Monitor services to ensure they remain effective

Our goal is to help individuals build strong support networks that promote independence, stability, and personal growth.

Helpful Organizations

Families may also find these organizations helpful when exploring services and advocacy resources:

• The Arc of New Jersey – Advocacy and support for individuals with intellectual and developmental disabilities.

• Autism New Jersey – Resources, education, and advocacy for individuals and families affected by autism.

• National Down Syndrome Society – Education, advocacy, and resources supporting individuals with Down syndrome.

These organizations provide valuable educational materials, family support resources, and advocacy services.
